I hereby affirm that the following statements are true for me:

• I am currently healthy.

• I do not have any heart problems, chest pain, palpitations or issues with my blood pressure.

• I have never suffered a stroke or any conditions related to poor circulation.

I understand that participating in wellness activities — including saunas, steam rooms, hot pools, cold plunge, swimming pools and spa treatments — carries inherent risks, including but not limited to dehydration, dizziness, loss of balance, and cardiovascular stress.

I acknowledge that:

• I am using the facilities voluntarily and at my own risk.

• I have disclosed all relevant medical information.

• DIP on the Roof is not liable for injury, illness, loss, or damage to persons or personal property.

• Some areas are not supervised by management or monitored by CCTV.

• I will follow all posted legal notice & terms of use, safety guidelines, staff instructions, and use all areas responsibly.

By booking, I acknowledge that I have voluntarily chosen to participate in DIP on the Roof activities. Before engaging in these activities - I have read, understood, and agree to the terms outlined in this liability waiver.

• I agree to the terms above and release DIP on the Roof, its owners, staffs, and affiliates from any and all liability.

• I consent to the use of any photographs or videos taken of me during the activities for DIP on the Roof promotional purposes, without expecting any compensation in return.
